Medicare Supplement Plans: Your Complete NJ Medigap Guide

If you're enrolled in Original Medicare (Parts A and B), you've probably noticed it doesn't cover everything. Deductibles, copayments, and coinsurance can add up quickly. That's where Medicare Supplement insurance—also called Medigap—comes in. These policies, sold by private insurance companies, help fill the "gaps" in Original Medicare coverage, giving you more predictable healthcare costs and greater peace of mind.

How Medicare Supplement Plans Work

Medigap policies work alongside Original Medicare, not as a replacement. When you receive healthcare services, Original Medicare pays its share first, then your Medigap policy pays its share of the remaining costs according to your plan's benefits. You can visit any doctor or hospital in the United States that accepts Medicare—there are no network restrictions with Medigap plans.

It's important to note that Medigap policies only work with Original Medicare. If you're enrolled in a Medicare Advantage plan (Part C), you cannot use a Medigap policy. You'll need to be enrolled in both Medicare Part A and Part B to purchase a Medicare Supplement plan.

Understanding the Standardized Plan Letters

Medicare Supplement plans are standardized by the federal government and identified by letters: A, B, C, D, F, G, K, L, M, and N. Each letter represents a different set of benefits, and these benefits are the same regardless of which insurance company sells the policy. For example, Plan G from one company offers the exact same coverage as Plan G from another company—only the premium and customer service may differ.

Here's what you should know about the most popular plans:

Plan G has become the most comprehensive option for new Medicare beneficiaries. It covers nearly all out-of-pocket costs except the Part B deductible.

Plan N offers solid coverage at typically lower premiums but requires small copayments for some doctor visits and emergency room visits.

Plans K and L provide partial coverage of Medicare cost-sharing and feature lower premiums, but you'll pay more out-of-pocket until you reach an annual limit.

High-deductible Plan G requires you to pay a deductible before coverage begins but offers much lower monthly premiums.

Note that Plans C and F are no longer available to those who became eligible for Medicare on or after January 1, 2020, though current enrollees can keep these plans.

Monthly Premium Considerations and Guaranteed Issue Rights

Medigap premiums vary based on several factors, including which plan letter you choose, where you live in New Jersey, your age, whether you use tobacco, and which insurance company you select. Some companies use community-rated pricing (everyone pays the same regardless of age), while others use issue-age or attained-age rating methods.

The best time to buy a Medigap policy is during your Medigap Open Enrollment Period—a six-month window that starts when you're 65 or older and enrolled in Medicare Part B. During this period, you have guaranteed issue rights, meaning insurance companies cannot deny you coverage or charge you more due to pre-existing health conditions.

You also have guaranteed issue rights in certain special situations, such as when you lose employer coverage, when your Medicare Advantage plan leaves your area, or when you move out of your plan's service area.

Choosing the Right Medigap Plan for Your Needs

Selecting the right Medicare Supplement plan depends on your individual circumstances. Consider these factors:

  • Your health status and expected healthcare needs: Do you visit specialists frequently or require regular medical care?
  • Your budget: Can you afford higher monthly premiums for more comprehensive coverage, or would you prefer lower premiums with some cost-sharing?
  • Your risk tolerance: Are you comfortable with some out-of-pocket costs, or do you prefer predictable expenses?
  • Your travel habits: Medigap plans often provide better coverage for travel within the United States compared to Medicare Advantage plans.

Remember, you're not locked into your first choice forever. You can switch Medigap plans, though you may need to answer health questions unless you have guaranteed issue rights.
