If you're enrolled in Original Medicare (Parts A and B), you've likely noticed that it doesn't cover everything. Deductibles, coinsurance, and copayments can add up quickly. That's where Medicare Supplement Insurance, commonly called Medigap, comes in. These policies are designed to help fill the "gaps" in Original Medicare coverage, giving you more predictable healthcare costs and greater peace of mind.
Medigap policies are sold by private insurance companies and work alongside Original Medicare. When you receive healthcare services, Original Medicare pays its share first, then your Medigap policy pays its share according to your plan's coverage. This coordination means you typically have fewer out-of-pocket expenses at the doctor's office or hospital.
It's important to understand that Medigap only works with Original Medicare—not with Medicare Advantage plans. You cannot have both a Medicare Advantage plan and a Medigap policy at the same time. Additionally, Medigap policies don't include prescription drug coverage, so you'll need to enroll in a separate Part D plan if you want medication coverage.
One of the most helpful aspects of Medigap is that plans are standardized by the federal government. Each plan letter offers the same basic benefits, regardless of which insurance company sells it. Here's what you need to know about the available plans:
Plans A through N each offer different levels of coverage. Plans F and G are among the most comprehensive, covering most out-of-pocket costs. However, Plan F is only available to those who became eligible for Medicare before January 1, 2020. For newer beneficiaries, Plan G offers similar comprehensive coverage.
Plans K and L offer lower monthly premiums but require you to pay a larger share of costs, up to an annual out-of-pocket limit. These plans typically cover 50% (Plan K) or 75% (Plan L) of most benefits.
Plans M and N provide moderate coverage with cost-sharing features. Plan N, for example, requires small copayments for some doctor visits and emergency room visits that don't result in admission.
Note that Plans C and F are no longer available to those newly eligible for Medicare, though current enrollees can keep their plans.
Medigap premiums vary based on several factors, including which plan letter you choose, the insurance company you select, your age, gender, tobacco use, and where you live in New Jersey. It's worth shopping around, as premiums for the same plan letter can differ significantly between companies—but remember, the benefits remain identical.
The best time to buy a Medigap policy is during your Medigap Open Enrollment Period, which lasts for six months and begins when you're both 65 or older and enrolled in Medicare Part B. During this period, you have guaranteed issue rights, meaning insurance companies cannot deny you coverage or charge you more due to pre-existing health conditions.
You also have guaranteed issue rights in certain special situations, such as when you're losing other coverage through no fault of your own. These protections are valuable, as outside of these periods, insurance companies in New Jersey can use medical underwriting to determine your eligibility and premiums.
Selecting the right Medigap plan depends on your individual healthcare needs, budget, and preferences. Consider these factors:
Remember that you can switch Medigap plans later, but you may need to go through medical underwriting unless you have guaranteed issue rights.
