If you're enrolled in Original Medicare (Parts A and B), you've probably noticed that it doesn't cover everything. Deductibles, copayments, and coinsurance can add up quickly. It's important to understand that Medigap only works with Original Medicare—not with Medicare Advantage plans. You cannot have both a Medicare Advantage plan and a Medigap policy at the same time. Medigap also doesn't include prescription drug coverage, so you'll need to enroll in a separate Part D plan if you want medication coverage.

Understanding the Standardized Medigap Plan Letters

Medigap policies are standardized by the federal government, meaning that a Plan G from one insurance company offers the same basic benefits as a Plan G from another company. The plans are identified by letters: A, B, C, D, F, G, K, L, M, and N. Each letter represents a different level of coverage.

Plans F and C are no longer available to people who became eligible for Medicare on or after January 1, 2020, though those who already have these plans can keep them.

Plan G has become one of the most popular choices because it offers comprehensive coverage, including Medicare Part B excess charges. It covers nearly everything except the Part B deductible.

Plan N is another popular option that offers slightly less coverage than Plan G but typically comes with lower monthly premiums. With Plan N, you may pay small copayments for doctor visits and emergency room visits.

Plans K and L offer lower premiums but require you to pay a percentage of costs until you reach an annual out-of-pocket limit.

Plans A, B, D, and M offer varying levels of coverage between these options, each with different combinations of benefits.

Monthly Premiums and Guaranteed Issue Rights

Medigap premiums vary based on several factors, including which plan letter you choose, which insurance company you select, and where you live in New Jersey. Insurance companies may use different pricing methods—some base premiums on your age, others on your location, and some use community rating.

The best time to enroll in a Medigap policy is during your Medigap Open Enrollment Period, which lasts for six months and begins when you're 65 or older and enrolled in Medicare Part B. During this period, you have guaranteed issue rights, meaning insurance companies cannot deny you coverage or charge you more due to pre-existing health conditions.

You may also have guaranteed issue rights in certain special situations, such as when you're losing other coverage or when an insurance company ends your coverage through no fault of your own. These protections are important because outside of guaranteed issue periods, insurance companies can use medical underwriting and may deny coverage based on your health status.

Choosing the Right Medigap Plan for You

Selecting the right Medigap plan depends on your individual healthcare needs, budget, and preferences. Consider these factors:

  • Your healthcare usage: Do you visit doctors frequently or have ongoing medical needs?
  • Your budget: Can you afford higher monthly premiums for more comprehensive coverage, or would you prefer lower premiums with some out-of-pocket costs?
  • Your travel habits: Do you need coverage outside your local area or even outside the United States?
  • Your risk tolerance: Are you comfortable with some uncertainty in costs, or do you prefer predictable expenses?

Remember that while you can't be turned down during your Medigap Open Enrollment Period, switching plans later may be more difficult if you develop health conditions.
