Medicare Supplement Plans Explained: Your NJ Medigap Guide

If you're enrolled in Original Medicare (Parts A and B), you've probably noticed that it doesn't cover everything. Deductibles, coinsurance, and copayments can add up quickly. That's where Medicare Supplement Insurance, commonly called Medigap, comes in. These private insurance policies work alongside Original Medicare to help cover many of those out-of-pocket costs, giving you more predictable healthcare expenses and greater peace of mind.

How Medicare Supplement Plans Work

Medigap policies are designed to fill in the "gaps" left by Original Medicare. When you receive healthcare services, Original Medicare pays its share first, and then your Medigap policy pays its share according to your plan's coverage. This coordination happens automatically—you simply show both your Medicare card and your Medigap card when you receive services.

It's important to understand that Medigap policies only work with Original Medicare, not with Medicare Advantage plans. You also cannot have both a Medigap policy and a Medicare Advantage plan at the same time. Medigap plans don't include prescription drug coverage, so most people with a Medigap policy also enroll in a separate Medicare Part D plan for medications.

Understanding the Standardized Plan Letters

One of the most helpful aspects of Medigap is that plans are standardized by the federal government. In New Jersey, insurers can offer plans labeled with letters: A, B, C, D, F, G, K, L, M, and N. Each letter represents a different set of benefits, and these benefits are identical regardless of which insurance company sells the plan.

For example, Plan G from one company offers the exact same coverage as Plan G from another company—the only differences are the monthly premium and customer service. Plans A and B offer basic coverage, while plans like F and G provide more comprehensive benefits. Plans K and L require you to pay a percentage of costs but have lower premiums. Plans M and N offer cost-sharing options that fall somewhere in between.

Note that Plan F is only available to those who were eligible for Medicare before January 1, 2020. For newer Medicare beneficiaries, Plan G has become one of the most popular choices due to its comprehensive coverage.

Monthly Premiums and Cost Considerations

Medigap premiums vary based on several factors. Insurance companies may price their plans using different methods: some base premiums on your age when you enroll (issue-age rated), others increase premiums as you get older (attained-age rated), and some charge everyone the same regardless of age (community-rated).

Your location in New Jersey, gender, tobacco use, and the specific insurance company you choose will also affect your premium. While plans with more comprehensive coverage typically cost more each month, they may save you money in the long run if you need frequent medical care. It's wise to compare not just the monthly premium but also what you'd pay out-of-pocket for deductibles and copayments under each plan.

Guaranteed Issue Rights and Enrollment Timing

The best time to buy a Medigap policy is during your Medigap Open Enrollment Period, which lasts for six months and begins when you're 65 or older and enrolled in Medicare Part B. During this period, you have guaranteed issue rights—insurance companies cannot deny you coverage or charge you more due to pre-existing health conditions.

You may also have guaranteed issue rights in certain other situations, such as when you lose employer coverage, move out of a Medicare Advantage plan's service area, or your Medicare Advantage plan leaves the area. Understanding these rights is crucial because outside of these periods, insurance companies in New Jersey can use medical underwriting, potentially denying coverage or charging higher premiums based on your health status.

Making the Right Choice for Your Needs

Choosing the right Medigap plan depends on your individual circumstances: your health status, budget, how often you visit doctors, and your risk tolerance for out-of-pocket costs. Some people prefer comprehensive coverage with higher premiums for maximum predictability, while others are comfortable with more cost-sharing in exchange for lower monthly payments.

Because Medigap plans are standardized, shopping around among different insurance companies for the best price on the same plan letter makes good sense. However, navigating all these options can feel overwhelming.
