It's important to understand that Medigap only works with Original Medicare—not with Medicare Advantage plans. You cannot have both a Medicare Advantage plan and a Medigap policy at the same time. Medigap also doesn't include prescription drug coverage, so you'll need to enroll in a separate Part D plan if you want medication coverage.

  • Plans F and G offer the most comprehensive coverage and are among the most popular choices. Note that Plan F is only available to those who became eligible for Medicare before January 1, 2020.

  • Plan N provides substantial coverage but requires small copayments for some doctor visits and emergency room visits that don't result in admission.

  • Plans K and L offer lower premiums but require you to pay a percentage of costs until you reach an annual out-of-pocket limit.

  • Plans A, B, C, D, and M offer varying levels of coverage between these options.

Each plan covers different combinations of costs like Part A coinsurance, Part B coinsurance, Part A deductible, Part B deductible, Part B excess charges, foreign travel emergency care, and skilled nursing facility coinsurance.

Premium Considerations and Guaranteed Issue Rights

Medigap premiums vary based on several factors, including which plan letter you choose, which insurance company you select, your age, your location in New Jersey, and whether you use tobacco. Insurance companies use different pricing methods—some base premiums on your age when you enroll, others on your current age, and some charge everyone the same regardless of age.

Your best opportunity to enroll in Medigap is during your Medigap Open Enrollment Period, which lasts six months and begins when you're both 65 or older and enrolled in Medicare Part B. During this period, you have guaranteed issue rights—meaning insurance companies cannot deny you coverage or charge you more due to pre-existing health conditions.

You may also have guaranteed issue rights in certain special situations, such as when you lose other coverage through no fault of your own, when your Medicare Advantage plan leaves the area, or in other specific circumstances defined by federal law.

Choosing the Right Medigap Plan for You

Selecting the right Medigap plan depends on your individual circumstances. Consider these factors:

  • Your budget: Balance monthly premiums against potential out-of-pocket costs
  • Your health status: How often do you visit doctors or need medical services?
  • Your doctors: Medigap plans work with any doctor who accepts Medicare
  • Your travel habits: Some plans offer foreign travel emergency coverage
  • Your risk tolerance: Are you comfortable with some cost-sharing, or do you prefer more comprehensive coverage?

Remember that while you can switch Medigap plans later, you may face medical underwriting unless you have guaranteed issue rights, which could result in higher premiums or denial of coverage.
