If you're enrolled in Original Medicare (Parts A and B), you've probably noticed that it doesn't cover everything. Deductibles, copayments, and coinsurance can add up quickly. That's where Medicare Supplement Insurance, commonly called Medigap, comes in. What Is Medigap and How Does It Work?

Medigap policies are sold by private insurance companies and work alongside your Original Medicare coverage. When you receive healthcare services, Original Medicare pays its share first, then your Medigap policy pays its share according to your plan's benefits. This coordination means you'll typically have fewer out-of-pocket expenses when you visit the doctor or need hospital care.

It's important to understand that Medigap only works with Original Medicare—not with Medicare Advantage plans. You cannot have both a Medicare Advantage plan and a Medigap policy at the same time. Medigap policies also don't include prescription drug coverage, so you'll need to enroll in a separate Part D plan if you want drug coverage.

Understanding the Standardized Plan Letters

Medigap policies are standardized by the federal government, meaning each plan letter offers the same basic benefits regardless of which insurance company sells it. In New Jersey and most states, you can choose from up to 10 standardized plans: A, B, C, D, F, G, K, L, M, and N.

Monthly Premiums and Pricing Considerations

Medigap premiums vary based on several factors, including which plan letter you choose, which insurance company you select, and where you live in New Jersey. Insurance companies use different pricing methods:

Community-rated policies charge the same premium to everyone regardless of age. Issue-age-rated policies base your premium on your age when you first buy the policy. Attained-age-rated policies increase premiums as you get older.

While monthly premiums are an important consideration, the cheapest plan isn't always the best value. Consider your overall health, how often you visit doctors, and your budget for both premiums and potential out-of-pocket costs.

Guaranteed Issue Rights and When to Enroll

The best time to buy a Medigap policy is during your Medigap Open Enrollment Period, which lasts for six months and begins when you're 65 or older and enrolled in Medicare Part B. During this period, you have guaranteed issue rights—insurance companies cannot deny you coverage or charge you more due to pre-existing health conditions.

You may also have guaranteed issue rights in certain special situations, such as when you're losing other coverage through no fault of your own, or if your Medicare Advantage plan is leaving your area.

Outside of these protected periods, insurance companies in New Jersey can use medical underwriting, potentially denying coverage or charging higher premiums based on your health status.

Choosing the right Medigap plan depends on your individual circumstances, including your health needs, budget, and preferences. Consider factors like how often you visit doctors, whether you travel frequently, and how much financial predictability you want.

Remember that all Medigap policies with the same letter offer identical benefits—only the premium and insurance company differ. This makes comparison shopping straightforward but important.
